Hey most of the 18+ ones come from Japan, and only a portion of them are 18+, most otome game are all-ages.
unfortunately there's hardly any otome game that has been translated, let alone the 18+ ones...

Hakuoki is *just* about to come out (or already is, I'm not sure which, but apparently the very first reviews were just published). That's the only real otome game in English for the PSP but it should be a good one!
