Its all around the other data like first/last name/ social/ DOB/ driver's license number. Any help would be great. Thanks!

If it's right next to "Driver's License Number", then it means "what state issued your driver's license"

I would assume it's the state that issued your drivers license, since every state has a different set of numbers.

It's probably by drivers license # - what state is your license from?
